Comprehensive Care Plans to be Emailed or Faxed

Dear Providers,

Please see the update from the Blue Cross and Blue Shield of New Mexico Community Centennial's Care Coordination team.

In a dual effort to be environmentally friendly and to enhance our contactless processes, our Care Coordination team will begin emailing and/or faxing Comprehensive Care Plans (CCP) to our providers. Previously, a paper copy of the CCP was mailed to the provider's office.

The CCPs are developed and reviewed with members and sent to providers on members' behalf. Please feel free to contact our Care Coordination Department if you have any questions. Tel: 877-232-5518, option 3, option 2.
